List healthcare needs we should be aware of, including: epipen, allergies, etc. 
  1. I understand that Kehiloft teenagers will be “babysitting” my child(ren) from 5:30- 9 p.m. at CKS on Saturday December 6th. There will be at least 2 adult supervisors as well with one having First Aid, CPR, Epipen administration and blood loss control certification.
  2. I understand that I will be called if there are any behavior issues that cannot be resolved by the teens and adults present.
  3. I understand pick-up is 9:00 p.m. I will pick up my child by that time.
